Dental Plan/Insurance & Patient Co-payments
If you have a dental benefit plan, we will help you determine the benefits you have available. In order to best assist you, it is important that you submit your dental plan information to us with your New Patient Paperwork no later than three business days prior to your scheduled appointment. This allows us time to contact your dental plan to obtain information on the benefits that you or your employer purchased from the company. In addition, you will want to bring a dental benefit book that your carrier should be able to provide or email us your policy in digital form. Our business team will be happy to help you understand your plan.
We will provide you with a detailed treatment plan that estimates your dental benefit and your copayment, which is due at the time of service.
Professional care is provided to you, our patient family, and not to your dental plan. You are responsible for all treatment fees. We will assist you every way we can to obtain your benefits for your treatment. As a courtesy we will file your dental claims on your behalf and respond to any questions in order for your claim to be processed. It is important that you provide all current dental plan information.
